Agreed and I've asked personal contacts too but a lot of people here do a lot of really random stuff. Wouldn't be surprised if we had medical billing or insurance experts here.
I do know there is a time limit to Medicare claims and if over the time limit there has to be documentation added as to why. Private insurance has different rules. Sounds like a billing or documentation error and I would def. inquire with your insurance and the provider.
A possible explanation is an internal audit on the insurance side came to the conclusion that they overpaid (lots of ways they reach such conclusions), and the relationship they have with all their providers is such that they can "take back" that payment, and so the provider then tries to collect from you.
Call your insurance company with dates of service in hand and ask them what happened. The provider will probably be less useful (depending upon the competency of the person you are asking), but ask them as well. Assuming it's not "fake" they are just trying to collect a legitimate bill and such actions by the insurance company are almost always legal
Remember you can always negotiate with the provider. Say the bill is $1000, explain to them that you don't have it, you lost your job due to Covid, your cat died, etc. and that if they will take $350 (and send you a reciept with "paid in full") you will give them your CC over the phone
First one explains a lot. Thanks. Problem is I actually have a copy of my plan from dates in question and no where in the plan are the services I received capped (and especially are not capped at the numbers they came up with).
Called insurance during normal business hours mult times only for the automated system to say "outside of normal business hours"... wtf...
